Body Lift. Circumferential lower body contouring.

Circumferential lifting of the lower body — addressing skin laxity across abdomen, hips, back, and outer thighs in a single comprehensive procedure, typically after significant weight loss.

The Procedure

A tummy tuck. Times three-hundred-sixty degrees.

A body lift — sometimes called a belt lipectomy or circumferential body lift — is the signature procedure of comprehensive post-weight-loss contouring. Where a tummy tuck addresses the front of the abdomen, a body lift extends that same principle circumferentially, removing excess skin from the abdomen, flanks, hips, back, and outer thighs in a single continuous procedure. The result is a comprehensive reshaping of the lower body that no other single procedure can accomplish.

This is a major reconstructive-cosmetic operation. The incision runs circumferentially around the low waist, hidden in the low bikini line in front and just above the buttocks in back. Excess skin and some fat is removed from the entire circumference, and the remaining tissue is repositioned to produce a smoother, more youthful lower body contour. The buttocks are typically reshaped or auto-augmented in the process; the outer thighs are lifted; and the front of the abdomen receives the same muscle repair and skin removal as a standard tummy tuck.

Body lift is most often part of a staged post-weight-loss contouring plan. It is rarely done in isolation without upper body work (breast lift, arm lift, or facelift/necklift) at some point in the sequence. Body lift is a major operation with strict candidacy criteria. The consultation is where fit is determined — not a checklist.

Significant post-weight-loss skin laxity

The primary indication. Bariatric surgery patients, major diet-and-exercise weight loss, or post-partum patients with truly circumferential skin excess. Standard cosmetic tummy tuck patients typically don't need a body lift.

Stable weight for at least 12 months

Weight loss should have plateaued for a year before body lift — ongoing loss changes what needs to be removed, and residual loss after surgery distorts the result.

BMI generally under 32

Higher BMI increases surgical risk substantially for a procedure of this scope. Some surgeons will proceed at higher BMI in specific cases; others will not.

Nutritional optimization

Bariatric patients specifically need adequate protein intake and appropriate vitamin/mineral status for wound healing. This is discussed in consultation and may require pre-operative workup.

Realistic understanding of scars

The circumferential scar is substantial. It fades over 12–18 months but never disappears. Patients who cannot accept the scar tradeoff are not body lift candidates — regardless of how good the contour result would be.

Realistic understanding of recovery

This is major surgery. Full recovery takes 3–6 months. Time off work is 4–6 weeks. Combined stages extend the total recovery timeline substantially.

Smoking status matters — and approach varies by surgeon

Every surgeon at NCSS strongly recommends smoking cessation for at least 6–8 weeks pre-operatively for cases with large incisions like this one — a longer window than most cosmetic procedures. Smoking substantially impairs healing at the long circumferential incision and dramatically increases complication risk, including wound breakdown and skin loss. Some of our surgeons, Dr. Nichter among them, will not perform surgery on patients who continue to smoke.

The Approach

Circumferential. Comprehensive.

Modern body lift technique addresses the lower body as a single continuous unit rather than a series of regional operations.

Circumferential incision

A continuous incision runs around the low waist, hidden in the low bikini line anteriorly and just above the buttock crease posteriorly. The incision is carefully planned pre-operatively with the patient standing.

Front (abdomen)

The abdominal portion functions as a full tummy tuck — excess skin removed, muscle wall repaired if diastasis is present, umbilicus repositioned, and the pubic area (mons) contoured.

Sides (flanks and hips)

Excess skin over the flanks is removed, tightening the waist and reducing the 'love handle' laxity that persists after significant weight loss.

Back (buttocks reshaping)

The posterior portion of the incision lifts and reshapes the buttocks. In select cases, tissue can be auto-augmented (moved from adjacent areas) for improved buttock projection without implants.

Outer thigh lift (included)

The circumferential lift naturally elevates the outer thigh tissue, addressing 'saddlebag' laxity that liposuction alone could not correct.

Concurrent liposuction

Very commonly combined for contouring of areas the excisional lift doesn't directly address.

Drain use

Drains are typically placed in the surgical field and remain in place until daily output diminishes below a specific threshold.

Because body lift can run 5–8 hours and involves substantial fluid shifts, this is one of the procedures where physician anesthesia and endocrinology on call matter most. Every case at NCSS meets both standards.

Recovery

Major surgery. Extended recovery.

Body lift recovery is measured in months, not weeks. Plan accordingly — and plan for help.

FIRST 24–48 HOURS

Discharged the day of surgery to a caregiver-supervised recovery setting. Compression garment worn continuously. Drains in place. Walking in a slightly-bent posture is expected — do not attempt to stand fully upright yet.

FIRST WEEK

Drains typically remain in place. No showering until drains come out. Compression continues. Substantial help needed with basic tasks. No lifting anything heavier than a coffee cup.

WEEKS 2–4

Progressive return to normal activity. Patients able to stand fully upright by week 3. Drains may still be in place through week 2–3. Return to sedentary work at 4 weeks depending on job demands.

WEEKS 4–6

Increasing light activity, initially walking. No core exercise, upper-body work, or bending yet.

WEEKS 6–8

Return to progressive exercise once your surgeon confirms complete healing of the circumferential incision.

MONTHS 3–6

Final swelling resolves and the incision continues to fade. Body contour finalizes its shape.

MONTHS 6–18

Scar maturation continues. Silicone scar treatments help. Final scar appearance apparent at 12–18 months.

LONG TERM

Results are durable with stable weight. Future significant weight changes will alter the result. Some patients pursue small refinement procedures years later if specific areas warrant it.
