Fat transfer to breast, refined and branded.
The California Breast Lift® is the registered name Drs. Larry Nichter and Jed Horowitz use for their refined approach to autologous fat transfer to breast. The underlying procedure is fat transfer — fat harvested from a donor site (typically abdomen, flanks, or thighs), purified, and re-injected into the breast tissue. What they have trademarked is the specific technique they developed at Pacific Center for Plastic Surgery, and the aesthetic result it consistently produces.
The word 'lift' in the name refers to the visual outcome, not the surgical approach. There is no traditional mastopexy incision. Instead, strategic placement of transferred fat at the upper pole of the breast creates the appearance of a lifted, more youthful shape — while simultaneously adding modest volume. The result reads natural in the specific light and lifestyle of Southern California: on the beach, at the pool, under swimwear. It is the shape a woman recognizes as her breast, restored.
For patients researching this procedure: what Drs. Nichter and Horowitz call the California Breast Lift® is what other surgeons (including Dr. Bednar on our roster) call fat transfer to breast. The clinical procedure is the same. The refinement, the trademark, and the specific aesthetic goal are theirs.
Who this procedure is designed for.
The California Breast Lift® is the right choice for patients seeking natural, implant-free enhancement with a lifted aesthetic.
Wanting modest enhancement with a lifted appearance
The signature aesthetic of this technique: a natural-looking half-cup to one-cup increase with upper-pole fullness that creates the visual effect of lift.
Preference to avoid implants
Patients who want the permanent, biologic option — no capsular contracture concerns, no implant replacement over time.
Modest ptosis, not severe
The upper-pole volume placement creates a lifted look for mild-to-moderate ptosis. Patients with severe ptosis may need a traditional mastopexy in addition to (or instead of) fat transfer.
Prior implant complications
Women who have had capsular contracture, rippling, or other implant issues and want a permanent, natural restoration.
Sufficient donor-site fat
Fat transfer requires harvestable fat from another region. Very lean patients may not have enough donor volume.
Realistic expectations about staging
A percentage of transferred fat is reabsorbed by the body. Best results often come from staged sessions rather than a single large transfer.
Every scheduled surgical patient is offered a free Rest-Assure pre-operative consultation with Dr. Rand Scott personally — medical history, medication planning, and discharge coordination before surgery day.
What makes this technique distinct.
The underlying procedure is fat transfer to breast — a three-stage single-session operation. What Drs. Nichter and Horowitz have refined and trademarked is the specific way each stage is performed to consistently produce the lifted aesthetic that names the procedure:
Stage one — the harvest
Gentle low-pressure liposuction from a donor site (abdomen, flanks, thighs). Aggressive liposuction damages fat cells and reduces graft survival; the harvest technique is optimized for cell viability.
Stage two — the purification
Harvested fat is processed to separate viable fat cells from oil, blood, and tumescent fluid. This step directly determines graft survival rates.
Stage three — the strategic transfer
This is where the California Breast Lift® technique diverges from generic fat transfer. Purified fat is layered into the breast tissue with specific attention to upper-pole placement — producing the appearance of lift alongside volume. The distribution pattern (many small aliquots rather than large pockets) allows each fat cell to establish blood supply and survive long-term.
Trademark protects the approach
Because the specifics of the technique are proprietary, the details are discussed during consultation with Dr. Nichter or Dr. Horowitz. Their published trademark protects the approach they developed.
Every California Breast Lift® procedure at NCSS is performed under physician anesthesia — no CRNAs, no exceptions — in our AAAHC-accredited operating rooms with endocrinology on call.
What to expect afterward.
Recovery from the California Breast Lift® is primarily driven by the donor-site liposuction, not the breast itself.
Discharged the day of surgery. Compression garment worn on the donor site. Multimodal pain protocol keeps discomfort manageable without heavy opioid use.
Bruising and soreness at the donor site (typically abdomen or flanks). Minimal restriction on the breast itself. Most patients return to desk work within 5–7 days.
Compression garment continues on the donor site. Progressive return to normal activity. No pressure on the breast tissue — specifically no underwire bras — while transferred fat establishes blood supply.
A percentage of transferred fat is reabsorbed by the body during this window — expected and normal. Final volume and shape settle around three months.
Fat cells that survive establish their own blood supply and remain permanently. Weight fluctuations will affect them the same way they affect fat cells elsewhere in the body. The trademark's design intent is a result that reads natural and lifted over time.
Every California Breast Lift® patient at NCSS is covered by our Rest-Assure program: physician anesthesia on the day of surgery, one nurse from arrival through discharge, and post-operative direct access to your surgeon — not an answering service.
The trademark holders. And their fellows.
The California Breast Lift® brand is used by Drs. Nichter and Horowitz. Their trained fellows also perform the technique.
